Dr. Horowitz is not a participating provider for any HMO, PPO or EPO. He is not contracted with Care California affiliated companies. He provides medical services based on a traditional fee for service model. Payment is due in full at the time of service unless prior arrangements have been made with the office in advance. The office will either provide patients with a charge slip that contains all information necessary to submit to their insurance for reimbursement or may submit charges to their insurance company on their behalf. Most insurance companies will reimburse the patient for a portion of the charges. It is the patient’s responsibility to contact their insurance company to determine the amount their company will reimburse for services rendered by a non-participating provider. The office accepts cash, checks and most major credit cards.
